Change conditions:

During the validity period of the hazardous chemicals business license and when the license is not renewed, the business unit shall go through the corresponding change procedures if the following changes occur: (if multiple changes are involved, only one application for change shall be filled in.)

LIST OF MATERIALS:

Ø Name of change unit:

The materials to be submitted by the business unit applying for changing the name of the unit are 3 items:

1. Application Form for Change of Hazardous Chemicals Business License;

2. A copy of the original hazardous chemicals business license;

3. A copy of the industrial and commercial business license after the change or the industrial and commercial advance change approval notice.

Ø Change unit type:

The materials to be submitted by a business unit applying for a change in the type of unit are 3 items:

1. Application Form for Change of Hazardous Chemicals Business License;

2. A copy of the original hazardous chemicals business license;

3. A copy of the industrial and commercial business license after the change.

Ø Change the person in charge of the unit:

The business unit that applies for the change of the person in charge of the unit shall submit 3 items of materials:

1. Application Form for Change of Hazardous Chemicals Business License;

2. A copy of the original hazardous chemicals business license;

3. Safety training certificate of the person in charge after the change.

Ø Scope of change permission:

First, the business unit to narrow the scope of licensed business, should submit materials for 7 items:

1. Application Form for Change of Hazardous Chemicals Business License;

2. A copy of the original hazardous chemicals business license;

3. A copy of the industrial and commercial business license;

4. Fire safety acceptance documents for buildings in operating and storage sites;

5. Proof of property rights or lease of operating and storage sites and facilities;

6. The main person in charge of the unit and the competent personnel, safety production management personnel and business personnel professional training certificate;

7. Safety management system and post safety operation procedures.

Second, business units that expand the scope of the license should reapply for processing, and the materials to be submitted are 8 items:

1. Application Form for Hazardous Chemicals Business License;

2. Safety evaluation report;

3. Fire safety acceptance documents for buildings in operating and storage sites;

4. Certificate of property right or lease of operation and storage place and facilities;

5. The main person in charge and the person in charge of the unit, the safety production management personnel and the professional training certificate of the business personnel;

6. Safety management system and post safety operation procedures;

7. A copy of the original hazardous chemicals business license;

8. Duplicate copy of business license.

Ø Change of business premises number:

1. Application Form for Change of Hazardous Chemicals Business License;

2. Relevant certification documents of house number change issued by the public security department;

3. A copy of the original hazardous chemicals business license;

4. A copy of the industrial and commercial business license after the change.
